Three hazards a typical upstream shift meeting covers — confined-space entry on a separator during turn-around, H2S and hydrocarbon exposure at the well-pad, and hot work near live piping. Each row is the brief the supervisor reads at the start of shift, then has the crew sign against.

Hazards

  • Separator vessel S-203 opening for internal inspection during turn-around
  • Residual H2S pockets flagged on last shift’s gas test — purge cycle fresh this morning
  • Two new contract entrants on the rescue roster; lead entrant on day two of a four-day rotation

Controls

  • Hot / cold work permit signed, attendant posted at the manway with continuous comms
  • LEL meter and 4-gas monitor on the entrant — sample every five minutes at chest height
  • Rescue team staged on deck, not on the ground — stretcher and SCBA pre-staged

PPE

  • Supplied-air respirator (Grade D) or SCBA inside the vessel
  • Harness with retrieval line tied off to the davit, never to a fixed point
  • FR coveralls (CAT 2) with arc-rated option if adjacent hot work is live
Hydrocarbon exposure

Hazards

  • H2S recorded 8 ppm at the test separator yesterday — under alarm, over comfort
  • Wind 12 km/h NW this morning — southerly workers on the road crew downwind
  • Flare stack lit but unstable pilot; risk of un-ignited release on cold start

Controls

  • Personal H2S monitor on every badge in the 50 m radius — bump test before entry
  • Wind check every 30 minutes; upwind muster point re-staged as the wind shifts
  • Fresh-air refuge tent with SCBA cached at the lease edge, radio check at 06:50

PPE

  • SCBA or escape bottle on every entrant within the radius
  • H2S-rated sense hood where continuous monitoring is impractical
  • Chemical-splash goggles + face shield over the hood whenever opening lines
Hot work near live piping
Hazard 03 · today

Hazards

  • Weld repairs on the east header — live product line 4 m above the weld
  • Recent rain left decking slick around the weld station; combustibles 12 m at the pad edge
  • Two grinders and a wire-feed unit sharing the same 30 A receptacle

Controls

  • Hot-work permit signed, fire watch posted within arm’s reach of the arc
  • Combustibles cleared 11 m (35 ft) around the station — gas test inside the envelope every 30 min
  • Lockout / tagout at the header isolation valve; bleed vent left open with the cap removed

PPE

  • Auto-darkening welding hood ( Shade 11 ) + FR jacket rated to the amperage
  • FR leathers over boots, no exposed laces in the arc radius
  • Cut-A cut-resistant gloves under the welding gauntlets for slag handling
